Becoming A New Patient

We accept most major insurance carriers, including Medicare.  We do not accept Medicaid, which includes Sunshine Health.  Please contact your individual insurance provider or your employer’s human resource department to confirm your in­ network benefits.  Rowan Tree Medical P. A., Dr. Jennifer Bartczak, or Dr. Myron Davidson, should be listed as “in­ network” providers, otherwise you may incur higher costs, than anticipated.  Please do this before completing the following online form below.

If one of our Doctors has been specifically referred to you, you are able to select them as your provider, while filling out the form below.

If you submit this form, we will contact you as soon as possible to schedule your first appointment. New patient appointments are typically scheduled 2­ – 3 weeks out. If you are unable to complete the application online, please contact the office at (954) 533­-5382 to coordinate the application.

Your first initial visit is for our physician to become acquainted with you, for you to discuss your needs, and discuss your medical history and family medical history, if any.  If you need an Annual Physical Exam, you may schedule this after you have completed your initial visit, unfortunately it cannot occur during your first visit.

FIRST APPOINTMENT:
Please come at least 15-30 minutes early.

“wellness visits/physicals” are not done on the first visit, they are ONLY done on your follow-up visit after you’ve established care and had labs.
